If you are an employer seeking reimbursement for a long service leave payment made directly to your employee, and you’re unable to provide proof of payment, your employee will need to complete and sign this form.
Once the form is completed, attach it to your reimbursement claim application in the Employer Portal, along with any other required documentation.
