Complete and submit a request for reimbursement if you are an Employer and wish to apply for a reimbursement of a long service leave payment made directly to and employee under:
- the Long Service Leave Act 1976
- a law prescribed by regulation e.g. Enterprise Bargaining Agreement (EBA).
